Episode 83 (Epilogue): Vaginal Birth After Cesarean Part II

We started the show tonight by talking to our intern, Lauren Brenzel, about her thesis research, which focused on control and empowerment in both the hospital and out-of-hospital birth setting. She shared a bit of her findings with us on air.

On tonight’s show, Laura and Carmela revisited the subject of vaginal births after Cesarean sections (or VBAC). Tonight, we delved a bit deeper into the experience of women who seek VBAC care. Our first guest tonight was Lisl Liang. Lisl is the the cofounder, President and Editor in Chief of SRQ Magazine, and the mother of two children--Griffin, her almost three year old son and Piper who celebrated her two month birthday last Friday. She is also the Chair Elect of the Sarasota Chamber of Commerce and recent keynote speaker for the Women's Leadership Council of the Boys and Girls Clubs of Sarasota County. Lisl shared both of her birth stories with us, and the learning and growing she did after her C-section with her first child, Griffin.  In particular, Lisl shared with us the lessons she learned in following her intuition and letting go of the idea of a perfectly planned birth, stating that “preconceived notions of what we want… can override instinct.” Lisl also highlighted the importance of self-advocacy, and learning to communicate needs with care providers.

Our next guest was Sharon Lytle. Sharon was introduced to midwifery in 1991 while working in a small community in Illinois. She followed in the footsteps of other local nurse by attending the University of Illinois at Chicago's midwifery program, earning a Master of Science in Maternal/Child Nursing with a Midwifery focus in 1999. Since that time, she has served women in both private practice as well as public health settings. A resident of Sarasota for two years, Sharon is happy to have recently joined Dr Sullivan, Dr Cohen, and nurse practitioners Marge Thiron, and Beth Hinkleman at Associates in Gynecology and Obstetrics. While interviewing with Dr's Sullivan and Cohen, one of the first questions on Sharon's agenda was "Do you offer VBAC?" Sharon discussed with us how individuals within her practice come to be informed about their options concerning VBAC, and also discussed issues of informed consent and active dialogue with patients on the subject of VBAC. Sharon encouraged individuals seeking VBAC care to ask lots of questions. Sharon also discussed some potential steps that could be taken in a general sense to increase VBAC numbers.

Our final guest tonight was Roanna Rosewood. Roanna is an award-winning international speaker, successful restaurateur, accomplished real estate investor, co-founder of BirthPlan Radio , executive action chair for Human Rights in Childbirth , and author of the new memoir, Cut Stapled and Mended: When one Woman Reclaimed Her Body and Gave Birth on Her Own Terms After Cesarean. She shared her motivations for writing her memoir.

Episode 82 (Epilogue): Tandem Nursing

Tandem Nursing refers to a mom who nurses multiple babies or children at the same time. Sometimes these children are the same age, such as twins, but sometimes these individuals are different ages, such as siblings. Cheryl, and tandem nurser herself, and Carmela discussed tandem nursing, with a focus on nursing siblings of different ages. They started by noting that each tandem nursing mother is different, just as their individual nursing relationship with each child is different

Our guest tonight was Jeanie DeLa. Jeanie is a parenting enthusiast, Certified Lactation Counselor, Certified Childbirth Educator, Car Seat Safety Technician, Infant Mental Health Specialist, and Licensed Mental Health Counselor. But her greatest achievement is being mother to Justin age 11 and fraternal twins Jason and Jamie age 8. Jeanie tandem nursed her twins, and walked us through some of the struggles she had when she first started nursing, and how she came through this difficult time to a successful tandem nursing experience. Jeanie and Cheryl discussed concerns that individuals may have about milk supply, and assured listeners that bodies can make enough milk to nurse multiple children.  The also stressed the importance of reaching out for help if individuals are struggling with nursing.  Jeanie also dispelled the myth that women cannot nurse when they’re pregnant, and noted that for the vast majority of individuals, nursing during pregnancy is safe.  We also discussed shifts in breast milk during pregnancy and after birth, and what this means for tandem nursing.

Cheryl then moved to a discussion of some of the potential frustrations that can be associated with tandem nursing, in particular a need for space that mothers may feel when nursing both a baby and a toddler. Jeanie suggested some steps to take in order to develop new nursing patterns with the older child to best meet the needs of everyone involved.  Cheryl and Jeanie also discussed “momma guilt” in nursing relationship, and how mothers can best manage by paying particular attention to the specific needs of each child, instead of just general ideas of what being the “best” mother is.

Next, Carmela and Jeanie discussed potential for germ and illness sharing in tandem nursing household, focusing on the fact that breast milk is a good boost for the immune system. Finally, Jeanie discussed how individuals with chronic low supply can best attempt tandem nursing, focusing in particular on releasing guilt associated with supplementation and cherishing beautiful breastfeeding moments that do happen as more than just filling bellies. We closed our show tonight with some listener statements on tandem nursing.

Report from Rwanda, Vol. 5

Hi Friends and Family:

I have been here 3 months now and all is well. The recent African GI disease was defeated with drugs.

We had a retreat Thursday and Friday about the program and all U.S. and Rwandans involved we are making progress. Progress comes slowly wherever you are. We are all doing what we want to do and it is working. This is just year one of the program. See more here.

The students and nurses and residents are learning "best practice". We are learning a lot as well.

Working decreasing maternal deaths and treating postpartum hemorrhage deaths.

We are all well. One of our colleagues though passed away suddenly so this was a sad month. The American community is very close. One is pregnant and I saw her. She will deliver in South Africa.

This is a time for grieving in Rwanda as they remember the genocide. They have events all month but not as many as in the past.

The people here are very nice except for those few who try to charge too much.

Took a trip to Burundi which is small country south of here. It was a needed R and R.

Today is Sunday and we go. The benches are wood and hard.  The country is very religious.

Here are some pictures!

Today in church. A call to pray.

We get CNN in our little apartment but a lot of the news recently has not been
good.

People coming and going bring stuff and take mail. Works well. We will fill up with our supplies in July.

Cases are still interesting e.g. Abdominal pregnancy and BOTH survived!

On Lake Tanganyika. When is this over? I can't swim.

Markets are colorful! People with light skin draw attention.

A Perinatal unit in Burundi. Nice nursing notes. No complaints about computers there.

A new Rwandan last week.
"If you push we will give you the rest of your Fanta!!
Students who just finished the rotation outside conference room. They here their oral exam on Wednesday.

Until next time. We miss you family and friends.

She also runs Full Spectrum Health , which celebrated its 25th anniversary this month.  Our show tonight focused on adrenal heath and adrenal fatigue. Eve gave us a rundown of what we mean when we talk about “adrenal health,” where the adrenal gland are located, what they do, and how their function affects our body as a whole. Eve described the importance of the adrenal glands for various hormone production, including DHEA, adrenaline, and cortisol. We also discussed the ways in which the modern world may promote constant biological reactions to stress, which while originally meant to benefit human in dangerous fight or flight situations, now may negatively impact them.

We moved to a discussion of what individuals can do to keep adrenal glands healthy, focusing specifically on the importance of sleep. Eve described to us the importance of maintaining a proper sleep schedule, and described some symptoms of problematic sleeping which can be representative of adrenal issues.  Eve also highlighted the importance of proper nutrition, particularly a protein heavy breakfast, and eating full and complete meals instead of grazing throughout the day. Eve also gave some advice for nutritional intake for pregnant individuals.

We then discussed the societal influence of pushes for productivity which can negatively affect individual’s propensity for self-care. Eve stressed that with proper rest and self-care individuals will be able to get more work done during a lesser period of time. Finally, Eve answered some of our listener questions, which focused on topics such as adrenal function in stressful life events, the relationship between kidney stones and adrenal health, and adrenal fatigue during pregnancy.

Episode 80 (Epilogue): Natural Birth in the Hospital

Tonight’s show focused on having a natural hospital birth, and steps that individuals can take in order to help facilitate this birthing plan. Laura and Ryan joined a variety of different guests tonight, including a local mother and a handful of labor and delivery nurses, to talk about natural hospital births. Our first guest tonight was Melissa Hembree. Melissa is a stay-at-home mom to two very happy children, who gave birth to both of them naturally in the hospital setting. Melissa shared with us each of her birth stories, which highlighted the importance of loved one support, relationship with care providers, and trust of the hospital staff. Melissa also encouraged women to speak up for themselves, by taking actions like asking questions and creating clear birth plan, in order to support personal empowerment in the birth setting.

Our next guest was Catey Brannan. Catey is an RN, a birth assistant, an LMT, and an improv comedian. Catey currently works at Desoto Memorial Hospital  as a labor and delivery nurse and at Birthway’s Family Birth Center  as a birth assistant and post-partum nurse. Catey discussed some differences and similarities that she sees working in both of these settings, and shared some points of overlap that she wishes to see take place. Mostly, Catey highlighted the need for women’s education (and access to education!) regarding birth and pregnancy. Catey also discussed the importance of care provider support for a hospital birth, focusing on the need to let every woman who is giving birth know that that is her special day.

We then talked to Judy Cavallero. Judy is currently the Clinical Manager of L/D, Antepartum, OB ECC and MBU at Sarasota Memorial Hospital. Judy discussed some of the exciting changes that are taking place at Sarasota Memorial, including the upcoming opening of their new maternity tower! She shared with us that Sarasota Memorial is including a bunch of new low intervention strategies, which the opening of this tower will facilitate. Our final guest tonight was Brandy Thomas. Brandy is a CNM who works out of Lakewood Ranch Medical Center. Brady discussed with us the role of midwives in the hospital setting, and individual’s potential preference for a midwife in the hospital setting if they are interested in a hospital birth.

We closed our show tonight with open discussion from all of our guests, in which we discussed the importance of open communication with hospital and care provider, personal education, and understandings of personal risk. Our guest encouraged listeners to interview providers and ask a lot of questions to make sure they have the right fit for them.

Episode 79 (Epilogue): Kids and Pets

We joined Carmela and Ryan to discuss the relationship between children and pets. Whether you’re brining a child’s first pet into the home, welcoming your first baby into the world and hoping your furry companion will welcome them too, or dealing already dealing with the relationship between your kids and your pets, a healthy, safe relationship between children and pets is fundamentally important. Our guest tonight was Dr. Gary Reinhardt. Dr. Gary is owner/operator of the Veterinary Center of Sarasota located at 311 South Washington Blvd. in downtown Sarasota. Dr. Gary graduated with honors from Michigan State University in 1974. He worked in a multi-Vet hospital in downtown Chicago, in a specialty clinic in northern California and worked and managed an emergency clinic in Berkeley California before he began taking care of our community pets in Sarasota in 1985.

We began with a discussion of pregnancy and dogs, focusing on the importance of setting boundaries and creating a safe space before welcoming a baby into the household. Some of the tips for dogs (and cats!) included exposes pets to baby furniture and toys beforehand in order to familiarize them, taking “Leadership Walks” with dogs, and how to introduce pets to babies. Dr. Gary also explained some safety concerns regarding pregnant mothers and illnesses that can be contracted from pets, particularly cats and their litter boxes, and some potential dangers from birds, rodents, and lizards. Carmela also questioned Dr. Gary about the notion that “Your pets are your children, until you have children, and then they’re your pet.”  Gary shared the importance of pet owners making sure they are letting their pets be pets, and not trying to make them people.

We then moved on to a discussion of bringing new pets into a home that already has children. Gary gave suggestions for what age it is best to bring children in the home, what age it is appropriate to expect children to be able to take on responsibility for pets, and what kind of responsibility you can expect from children at these ages. Dr. Gary also generally discussed which dogs are “good for kids,” and why.  Dr. Gary also provided some recommendations for children and dog safety training in the area. Dr. Gary also discussed what to do when a dog bites a child, and the potential dangers of squeaky toys!

We then discussed helping kids through grieving the loss of pets. Dr. Gary suggested seeking out books (including When a Pet Dies by Mr. Rogers), and finding specialists in pet loss if necessary.  We closed with some questions from our listeners
